This form is only valid for faulty items, returns for up to 30 days from invoice/purchase date. All tags and packaging must be provided otherwise returns may not be processed.     A completed Returns form will be emailed to you (please check your junk/spam folder). Please print and form MUST BE attached to the packaging in order to process the returns efficiently. Without the attached form, we are unable to process a refund for the items returned.
